has been involved with esthetic dentistry since he graduated from Chiang Mai University in 1980. One of the true pioneer in his field, he enjoys giving lectures extensively all over Thailand and Internationally especially in South East Asia, also appointed visiting professor in many leading universities. He is a Diplomate of the Thai Board of Operative Dentistry and member of the board examiner of The Royal College of Dental Surgeons of Thailand. He is now Esthetic Dentistry Specialist in Restorative Dentistry Post-Graduate study program and Implant Dentistry Post-Graduate study program Chiang Mai University. Apart from his academic background he also interesting in Photography and Dental Photography is his passion since 1995.

Dr.Pokpong Amornvit

Is graduated DDS from Mahidol University, Thailand and MS in Maxillofacial Prosthetic from same Uni versity. He also completed Certificate in Implant Dentistry from UCLA. He received Ph.D. in Oral Health Science, and his research focused in digital dentistry. He gives lectures in his own course at Connext, Bangkok and also is a Part-time Instructor at Maxillofacial Prosthetic Program and Master in Implant Dentistry Program in Faculty of Dentistry, Mahidol University. He maintains his private practice at The S Clinic and focuses on esthetic and digital dentistry.

He integrates the knowledge of prosthetic dentistry, implant dentistry, clear aligner and digital dentistry in his practice. Dr. Pokpong is a developer of “PPFACEDESIGN” which uses the digital workflow with a philosophy of a digital facial driven design and digital smile design.This concept considers the importance of facial profile, proportions, harmony,esthetics oral rehabilitation using digital smile design to make esthetically pleasuring face. Dr. Pokpong developed PPFACEBOW,which is a digital facebow to utilize in treatment. He lectures nationally and internationally about Digital Dentistry and PPFACEDESIGN. He has published numerous papers in national and International Journals. Dr. Pokpong is a member of Dental Association of Thailand,Thai Prosthodontic Association, American Academy of Maxillofacial Prosthetics,American Academy of Dental Sleep Medicine, Maxillofacial Rehabilitation Society of Thailand, and European Aligner Society.

Implant Smile Design.

Recently, advancement and evolution in the digital technology in term of scanner and design software has led to the disruption in the treatment modalities in dentistry, especially implant dentistry. PPFACEDESIGN is the Facial driven design concept, uses a digital workflow with a philosophy of facial driven design and smille design by using multiplatform of design software. This focuses on design the prosthesis, tooth and implant in the ideal position to achieve an ideal esthetic and function. The Facial profile, proportion, harmony are considering, planning and treating with multidisciplinary approach for maiking the eathetically pleasuring face. In addition, the proper occlusion is correct with digital tools to create ideal occlusal scheme to repostioning of condyle in a favorable position. This presentation is focusing on utilized PPFACEDESIGN concept to design smile in implant which making the treatment much easier, faster and predicable.

เมื่อพูดถึงการปลูกกระดูกในฟันหน้า เชื่อว่าคุณหมอหลายๆท่านก็คงกังวลว่าทำออกมาแล้วจะสวยไหม รากเทียมจะรอดไหม ปัจจัยที่สำคัญที่สุดคงหนีไม่พ้นการวางแผนการรักษาที่ดีร่วมกับการเลือกวิธีที่ใช้ในการปลูกกระดูกรวมถึงวัสดุทดแทนกระดูกให้เหมาะสมในแต่ละสถานะการณ์ ลองมาดูกันว่าเทคนิคการปลูกกระดูกที่ผมใช้เป็นอย่างไรบ้าง เริ่มกันจากเคสง่ายๆชิวๆไปจนถึงเคสแบบเห็นแล้วอยากส่งร้านอื่น และมาดูผลการรักษากันว่าทำแบบไหนปัง ทำแบบไหนพัง แล้วพบกันนะครับ

Research

My principal research interests lie in the field of dental implantology, tissue engineering, specializing in bone substitute, soft tissue management and guided bone regeneration.I am currently investigating the potential of digital technology for bone tissue regeneration, soft tissue regeneration using bioengineered materials with or without incorporation of stem cells or growth factors. Ongoing research are including a series of clinical trials on novel biomaterials for alveolar ridge preservation, bone graft substitutes and soft tissue augmentation.

ปัจจุบันการใส่รากฟันเทียมถือเป็นอีกทางเลือกหนึ่งของการใส่ฟันทดแทนที่กำลังได้รับความนิยมเพิ่ม มากขึ้นเรื่อยๆ แต่อย่างไรก็ตามอายุการใช้งานที่ ยาวนาน และความสวยงามของรากฟันเทียมนั้นขึ้น อยู่กับหลากหลายปัจจัย เป็นที่ทราบกันดีว่าในระยะ เริ่มแรกปัจจัยเรื่องคุณภาพ และปริมาณของกระดูก มีความสำคัญต่อการยึดติดของรากฟันเทียมแต่ในระยะยาวนั้นความแข็งแรงและสภาวะสุขภาพของเนื้อเยื่อที่รองรับรากฟันเทียมจะมีความสัมพันธ์โดยตรงกับความหนาของเหงือก (Gingival thickness) และขนาดความกว้างของเยื่อบุผิวเหงือกชนิดที่มี เคอราทิน (Keratinized gingiva) ที่เพียงพอ ตลอดจนการดูแลรักษาความสะอาดและการกำจัดคราบจุลินทรีย์ของผู้ป่วย การมีความหนาเหงือกที่ บางอาจส่งผลต่อความสวยงามบริเวณรากฟันเทียมและอาจส่งผลต่อการละลายของกระดูกรอบราก ฟันเทียมภายหลังการใช้งานไปได้ และมีการศึกษา พบว่าการมีปริมาณเนื้อเยื่อที่มีเคอราทินน้อย รอบรากฟันเทียมนั้นมีความสัมพันธ์กับปริมาณคราบจุลินทรีย์์ที่สะสมและการอักเสบต่อเนื้อเยื่อ จะเห็นได้ว่าการฝังรากฟันเทียมให้มีความสำเร็จได้ในระยะยาวจะต้องมีการประเมินสภาพเหงือกบริเวณที่จะฝังรากฟันเทียมก่อนเสมอว่าเหมาะสมหรือไม่ การทำศัลยกรรมปลูกถ่ายเหงือกเพื่อ เพิ่มความหนาของชั้นเนื้อเยื่อเกี่ยวพัน หรือ เพื่อเพิ่มความกว้างของเหงือกให้มีเคอราทิน จึงเป็นหัตถการที่ต้องอาศัยศาสตร์และศิลป์ที่ เกี่ยวข้องโดยตรงกับงานทันตกรรมรากเทียม ในการบรรยายครั้งนี้จะเป็นการนำเสนอหลักการ แนวคิดข้อบ่งชี้ข้อควรพิจารณาในการเลือกแนว ทางในการรักษาการทำศัลยกรรมปลูกถ่ายเนื้อ เยื่อเหงือกเพื่อเพิ่มความหนาของเหงือกและเพิ่มความกว้างของเหงือกที่มีเคอราทินเทคนิคการ รักษา และโอกาสสำเร็จ แนวทางการรักษาใน บริเวณขอบเขตความสวยงาม (Esthetic zone) และในบริเวณขอบเขตที่ไม่เน้นความสวยงาม (Non-esthetic zone) ภาวะแทรกซ้อนที่อาจ จะเกิดขึ้นรวมถึงวิธีการจัดการการใช้เนื้อเยื่อ ปลูกถ่ายทดแทนการตัดเนื้อเยื่อจากผู้ป่วยเองผลการรักษาในระยะยาวรวมถึงการนำเสนอราย งานผู้ป่วย เพื่อให้ได้ผลการรักษาที่ดี และมี ประสิทธิภาพ

ปัจจุบัน การบูรณะฟันหน้าบนในผู้ป่วยที่สูญเสียฟันไปด้วยรากฟันเทียม ได้รับความนิยมอย่างกว้างขวาง แต่จำนวนความต้องการการรักษาที่เพิ่มขึ้นมักติดตามมาด้วยความเสี่ยงในความล้มเหลว ผลข้างเคียงที่ ไม่พึงประสงค์หลายอย่าง โดยหลักๆคือแผลผ่าตัดมีการแยก แตกออก มีความช้ำ ความบาดเจ็บ การติด เชื้อหลังการผ่าตัด ซึ่งก่อให้เกิดการยุบ การหดตัวของเนื้อเยื่อ ทั้งส่วนของกระดูก ( Bone ) และเหงือก ( Soft tissue ) ฟันเทียมด้านบนจึงขาดความสวยงามและไม่มีส่วนของเหงือกมารองรับ ดูลักษณะเป็นฟัน ที่ยืนยาว หรืออาจเห็นแกนเงาดำๆของโลหะโผล่ออกมา Microsurgery จึงเข้ามามีบทบาทสำคัญในการแก้ไขปัญหาที่เกิดขึ้น ด้วยอุปกรณ์ต่างๆที่มีขนาดเล็กลง มีความเข้ากันได้กับเนื้อเยื่อมากขึ้น ทำให้แผลหายง่ายและปากแผลปิดไวขึ้น ส่งผลให้ความสำเร็จในการ ผ่าตัดเพิ่มขึ้นเป็นที่น่าพึงพอใจของทั้ง ผู้ป่วย และ ทันตแพทย์ผู้ให้การรักษาเองด้วย

Dental implants are made out of titanium alloy. The expected outcome following dental implant placement surgery is a rigid stability due to osseointegration, bone remodeling phenomena at bone/implant fixture interface and, a mucosal seal which is characterized by the formation of thin junctional epithelium layer covering underlying connective tissue at alveolar crest. As number of individuals carrying dental implant supported restorations is increasing, the prevalence of peri-implant diseases following ≥5 years in function is now ranging from 20-40% of patients (10% of implants). In general, inflammatory responses affecting peri-implant tissues following mechanical loading were classified as peri-implant diseases; mucositis is defined as inflammation limited to soft tissue while implantitis means peri-implant bone loss. A prevalence of mucositis around 43-65% and that of implantitis as 13-26% was recently reported. Thus, as a clinician, it is crucial to understand the nature of the diseases, preventive measures, and any therapeutic strategies to prevent a failure of the dental implants for a long-term treatment success. This session will summarize the current knowledge of the peri-implant diseases and elucidate some treatment approaches that were demonstrated to be effective in peri-implant disease management.

Received her D.D.S. degree, graduating at the top of her class from the Srinakharinwirot University in Bangkok, Thailand. Dr. Kongkiatkamon continued her training at the University of Illinois in Chicago where she completed a Prosthodontics and Implant Dentistry Residency as well as a Post-Graduate Specialty Program in Prosthodontics.She then went on to continue her training at MD Anderson Cancer Center in Houston, Texas where she completed her fellowship in Maxillofacial Prosthetics and Dental Oncology in 2012.Dr.Kongkiatkamon is specialized in cosmetic dentistry, digital approach to planning computer-guided surgery and cad -cam abutment design. Her passion in dental technologies led in her in depth in knowledge and better application of digital implant dentisty. Dr. Kongkiatkamon is a Diplomate of the American Board of Prosthodontics. She was awarded the Young Star of the American College of Prosthodontics. She has lectured nationally and internationally in the field of Implant Dentistry. She is currently a part-time instructor limited Digital Dentistry and Implant dentistry in several universities in Thailand.

Provisional restoration not only intends to protect the prepared tooth, but also to maintain the tooth position. Gingival health and position surrounding the area during the provisional phase are just as important for optimal results. These conditions provide the ideal environment for the esthetic evaluation and eventual cementation of the final restoration. Since the provisional restoration serves as a prototype of final restoration. If treatment of a complex case is handles without the ability to provide high-quality provisional restorations, treatment may spiral out of control, become highly stressful and ultimately lead to premature failure of the final restorations. Provisional restoration is the key to success especially in the anterior region. There are many kinds of provisional restorations which we will cover in the lecture.

Implant at anterior zone is one of the most challenging situation for many implantologist, since number of conditions could be occurred such as different bone axis VS implant axis, limit mesio-distal width, lack of keratinized mucosa, vertical bone loss or bone concavity at the labial area which lead to the narrow bucco-lingual bone width. There is not only one way of the treatment for all these situations, while many of technic were introduced and reported such as the use of angulate implant technic, customized CAD/CAM abutment design, soft tissue management, vertical or horizontal bone augmentation using guided bone regeneration technic, small or reduced diameter implant or using digital technology for implant surgery: computer assisted implant surgery (guided surgery or navigation system) In this lecture, we will go through the evidence-based related the treatment in the situation of lacking of bucco-lingual bone. Different situation and management will be raised and demonstrated in order to share the experience of the treatment. Moreover, discussion of the long term success of each technics will be also addressed.

He was appointed to be an assistant professor at Louisiana State University, School of Dentistry and maintained his intramural practice focus in cosmetic and implant dentistry. He has received Golden Apple Awards from senior dental classes at LSUHSC as the outstanding instructor. His research and publications focus on cosmetic dentistry and ceramics. He was appointed to be reviewer board of The Journal of Prosthodontics til 2016. He has his own private practice in Bangkok names NP Dental Clinic.
